Risk factors for pneumonia associated with stroke in elderly patients：a Meta-analysis

AbstractObjective：To investigate the risk factors of pneumonia associated with stroke in elderly patients by Meta-analysis.Methods：Articles related to risk factors of pneumonia in elderly stroke patients were retrieved from PubMed，EMbase，Web of Science，the Cochrane Library，CNKI，CBM，WanFang Database and VIP.The search period was from the establishment of the database to July 30，2022.Literature was screened independently by two researchers，data were extracted，quality was evaluated according to NOS，and Meta-analysis was performed using RevMan 5.4 software.Results：A total of 16 articles were included，involving 3 758 elderly stroke patients，including 1 197 cases in the case group and 2 561 cases in the control group.The results of Meta-analysis showed that the risk factors for pneumonia associated with stroke in elderly patients were：age≥65 years old（OR＝1.17，95％CI 1.08-1.27，P＜0.000 01），long-term bed rest（OR＝2.35，95％CI 1.35-4.08，P=0.002），malnutrition（OR＝9.93，95％CI 5.23，18.86，P＜0.000 01），smoking history（OR＝3.39，95％CI 2.80-4.10，P＜0.000 01），diabetes history（OR＝3.07，95％CI 1.67-5.62，P＝0.000 3），pulmonary disease history（OR＝4.17，95％CI 3.28-5.31，P＜0.000 01），disturbance of consciousness（OR＝6.88，95％CI 4.28-11.06，P＜0.000 01），NIHSS score≥15（OR＝4.85，95％CI 2.54-9.25，P＜0.000 01），dysphagia（OR＝3.25，95％CI 2.69-3.92，P＜0.000 01），bulbar palsy（OR＝5.09，95％CI 2.73-9.48，P＜0.000 01），preventive anti-infection（OR＝3.04，95％CI 2.72-3.40，P＜0.00001），indent gastric tube（OR＝6.99，95％CI 5.30-9.23，P＜0.000 01），mechanical ventilation（OR＝2.26，95％CI 1.67-3.08，P＜0.000 01）.Conclusion：The available evidence shows that age≥65 years old，long-term bedridden，malnutrition，smoking history，diabetes history，pulmonary disease history，disturbance of consciousness，NIHSS score≥15，dysphagia，bulbar palsy，preventive anti-infection，indent gastric tube，mechanical ventilation are the main risk factors for elderly patients with stroke associated pneumonia.
